Athletics Forms
All athletes are required to have the PIAA Comprehensive Pre-Participation Physical Evaluation Form filled out prior to tryouts. This form only needs to be completed once in a twelve (12) month period as long as the athlete did not suffer a major injury or illness throughout the twelve (12) month period. This form may not be completed earlier than July 1st and shall be effective, regardless of when performed during a school year, until May 31st.
If the athlete suffers a major injury during the season, the recertification form (Sections 5 and 6) must be filled out by a doctor before the athlete will be permitted to return to play.
All parents of athletes are required to fill out Section 5 of the PIAA Comprehensive Initial Pre-participation Physical Evaluation Form every time the athlete tries out for a sport.
Athletes will not be eligible to tryout or practice with a team until this form is completed. (This could effect the athlete's chances of making a team. Coaches do not need to give individual tryouts at a later date to athletes who did not have their physical forms completed on the first days of tryouts or practice.)

Athletic Fee
In order to participate in any sport, players must pay a $100 athletic fee per sport. This money is due on the first day of tryouts and will be collected by the coach. No athlete will be allowed to participate in the tryout until the fee is paid. The athletic fee will be returned to any athlete who does not make the team.
