Athletics Forms

All athletes must have the following in order to try-out for any sport at Nazareth Academy High School. All forms and monies are due to the Athletic Trainer before the dates listed below.

1. PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) Form

2. NAHS Athletic Fee ($160 per sport*).

*This includes a $30 Sports Banquet Fee for each student athlete.

piaa comprehensive initial pre-participation physical evaluation (cippe) form

All athletes are required to have a PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) Form prior to tryouts. This only needs to be completed once in a twelve (12) month period as long as the athlete did not suffer a major injury or illness throughout the twelve (12) month period. This form may not be completed earlier than July 1st, 2010 and shall be effective, regardless of when performed during a school year, until May 31st, 2011.

If the athlete suffers a major injury during the season, the recertification form (Section 6) must be filled out by a doctor before the athlete will be permitted to return to play. Athletes will not be eligible to tryout or practice with a team until this form is completed. This could effect the athlete's chances of making a team -- coaches do not need to give individual tryouts at a later date to athletes who did not have their physical forms completed on the first days of tryouts or practice.

Any athlete already medically cleared to play, and playing multiple sports in a school year, will be required to turn in Page 5 of the PIAA packet, the Parent Supplemental Health History, for each subsequent sport that school year. For example, if your daughter played a winter sport, she would need to submit page 5 and the athletic fee in order to be eligible to try out for a spring sport.

As part of NAHS' new procedures, all forms for tryouts (all seasons) will be due prior to the tryout dates for each season. All forms MUST be received by the school's Athletic Trainer for the seasons as follows:

Fall 2010: August 9, 2010
Winter 2010-11: November 8, 2010
Spring 2011: February 23, 2011

Anyone not having the required PIAA forms in by the corresponding due date listed above may jeopardize their eligibility to tryout.

athletic fee

In order to participate in any sport, players must pay a $160 athletic fee per sport. The athletic fee will be returned to any athlete who does not make the team.

emergency athletic form

All athletes are required to mail in to the Athletic Trainer a completely filled out copy of the Emergency Athletic Form. Should in the course of a season or school year, any of the pertinent information change (new allergies or injuries, change of insurance information), it is the parent's responsibility to complete an updated version of the form and turn it into the Athletic Department.
