We are excited to announce that we are now offering the convenience of online registration through partner company, FamilyID. This will be the ONLY way that forms will be accepted. You can log into FamilyID at any time to add your information and to check your registration(s).

FamilyID is a secure registration platform that provides you with an easy, user-friendly way to register for our programs, and helps us to be more administratively efficient and environmentally responsible. When you register through FamilyID, the system keeps track of your information in your FamilyID profile so you enter your information only once for multiple uses, multiple family members and multiple teams.

Before Your Register

The only forms that need to be printed and handed or mailed in are the PIAA Physical Exam (sections 5 & 6). Please note: A current physical must be on file with the athletic trainer. Physicals cannot be performed prior to June 1st of the upcoming school year and will expire, regardless of when preformed, on May 31st of that school year. These pages can be mailed to:

Information Needed to Register

It will be helpful to have the following information handy to allow for quick and accurate completion of your online registration.

  • Doctor’s contact information
  • Health Insurance information including member ID
  • Current medications and prescribing doctor
  • Valid credit card or bank account information to pay the athletic fee through paypal

A parent/guardian can register an athlete here.

Follow these steps to complete the registration:

  1. Under the "Programs" title, click on the link for the program you want to sign up for. Scroll to bottom of page to either Sign Up as a Family or Individual if this is your first time using FamilyID, or Log In if you already have a FamilyID account.
  2. Sign Up for your secure FamilyID account by entering your family name, email address and password. You will receive an email with a link to confirm your new account. (If you don’t see the email, check your spam or junk folders.)
  3. Click on the link in your email confirmation, which will log you in to FamilyID, and then complete the information requested on the registration form. When you have completed the form, click the "Save & Continue" button.
  4. Nazareth Academy will now be taking on-line payments for the athletic fee! Click on “Pay Online”, and you will be directed to PayPal where you can pay with a PayPal account or with a debit or credit card. Once completed, you will receive an email from FamilyID confirming your registration and an email from PayPal confirming your payment.

If you need assistance with your registration, please call Family ID at 888-800-5583 x1 or email support@familyid.com. FamilyID also offers online chat during business hours and a support center at https://familyid.desk.com. Support is available 7 days a week and messages will be returned promptly. Please DO NOT call Nazareth with registration issues.
