By marking this box, I attest that I have read, understood, and accept the terms laid out in the 2024 NAHS Athletic Summer Waiver as outlined below.

 

I hereby release Nazareth Academy High School, the Commonwealth and the Department of Welfare from any and all claims arising out of participation in the recreational activities at Nazareth Academy High School, 4001 Grant Avenue Philadelphia, PA 19114. The undersigned, intending to be legally bound, does hereby release and forever discharge at Nazareth Academy High School, the Commonwealth and the Department of Welfare, its employees, officers, successors, heirs, executors and administrators, of actions and cause of action, suits debts, dues, account, bonds, covenants, contract, agreement, judgments, claims and demands whatsoever in law or equity, including attorney’s fees regarding such activity, I release Nazareth Academy High School, the Commonwealth and the Department of Welfare from claims arising out of any incidents involving personal injury to me/my daughter including, but not limited to, injury in any way related to any recreational activity at Nazareth Academy High School which I/my daughter have/has ever had, or which my/her heirs, executors, administrators, successors or assigns have ever had, prior to the date of this agreement. It is agreed and understood that permission to share the premise may be revoked at the discretion of Nazareth Academy High School with or without notice.
